Job Summary: The Resident Care Nurse ensures delivery of the highest level of care to residents of United Pioneer Home. The position is responsible for assessing each resident’s condition, coordinating care and implementation of services, and serving as the primary contact for family members and medical practitioners. The position also serves as MDS coordinator and works closely with the Director of Nursing and direct care staff to ensure compliance with all state and federal licensing regulations.
Qualifications
- Graduate of an accredited Registered Nursing program
- Minimum of one year of long-term care experience
- Charge nurse and/or supervision experience
- Previous MDS experience preferred
- Knowledge of ICD10 coding preferred
Licensure/Certifications Required
- Current licensure in the State of Wisconsin as a Registered Nurse (RN)
- Current CPR certification
- BA or BS degree preferred
Nursing Care Functions
- Works with Social Services and other staff to coordinate resident admissions and discharges. This may involve screening, completing care plans and other documentation, educating and communicating with families and direct care staff, and coordinating care with outside resources.
- Completes discharge summaries and recapitulation of stay.
- Performs continuous review and revision of each resident’s written plan of care.
- Assists with the coordination of laboratory services including collection of blood, urine and other specimens.
- Acts as a liaison with resident’s physician, providing timely updates, obtaining orders, entering orders into the resident’s medical record and plan of care, and working to ensure accurate implementation.
- Assists with physician/NP rounding including preparation, assistance during rounds and notifying family representatives of changes.
- Provides on-call support as directed.
- Consults with DON on nursing problems and interpretation of nursing home policies to ensure resident needs are met appropriately and timely.
- Oversees and assists with the monitoring and assessment of acute or injured residents to optimize health and comfort. This includes subjective and objective data collection, documentation review and initiating a review of the plan of care.
- Initiates and resolves follow-up and problem charting as needed.
- Assists with the immediate and thorough investigation of all suspected/reported abuse, neglect and/or misappropriation of property.
- Completes post-fall investigations and updates resident plan of care accordingly.
- Completes RN assessment and documentation of injuries of unknown source and other skin injuries.
- Helps facilitate and leads resident care conferences.
- Actively participates with facility Interdisciplinary meetings (IDT) and quality assurance meetings.
- Completes the MDS/RAI process as determined by facility policy and state and federal regulations.
- Performs direct care related activities as needed or assigned.
- Participates in the development, implementation and evaluation of the facility Quality Assurance Committee.
- Upholds the United Pioneer Home’s mission and ethical standards.

Staff nurses will plan, direct and carry out shift routines in the assigned nursing unit and observe, direct and evaluate the activities of personnel as the person “in charge” according to the Nurse Practice Act.
Qualifications: Current licensure in the State of Wisconsin, current CPR registration, previous long-term care experience or ability to learn
