UNITED PIONEER HOME ADOPT - A - TREE

United Pioneer Home is building a new skilled nursing home and assisted living on Butternut Avenue in Luck, Wisconsin.  UPH is doing an Adopt-A-Tree Program for the new building site.

A donation of $350 buys a tree and engraved marker.  Checks can be made payable to United Pioneer Home. Forms are available at the Business Office or you may view/print Application here.  Completed forms and payments can be mailed to United Pioneer Home, 210 East Park Avenue, Luck, WI 54853.Engraved Marker 6x4 in.
